What Medical Documents Do I Need?

The form alone is never enough. Your file must include:

  • A medical report covering your history (symptoms, age at onset, course of the condition, when treatment started), findings on physical examination, your physician's interpretation of symptoms and test results, a diagnosis based on current internationally accepted criteria, and evidence of ongoing follow-up.
  • The prescribed substance by generic name, with dose, route, frequency and duration.
  • Relevant laboratory, imaging and other test results, as copies of the original reports where possible, including documents from the physician who first diagnosed you.
  • An explanation of why permitted alternatives aren't suitable.

Your physician can use the WFDF TUE Support Letter Template as the basis for the medical report. WADA publishes TUE Checklists and Physician Guidelines for common conditions (asthma, ADHD, diabetes, inflammatory bowel disease, male hypogonadism, transgender athletes and others). Give your physician the ones for your condition before they write the report. You pay for any tests, reports or translations your application needs. WFDF doesn't cover these costs.
