Why Do Applications Get Returned?
The most common reasons WFDF sees: the form was sent without supporting medical documents; an outdated form was used; the condition-specific Checklist items were missing (most often for asthma and ADHD); the form was signed by someone other than a physician; or documents in another language came without a physician's translated summary. Check all five before you send.
